Sometime back, the Journal of American Medicine Association (JAMA) wrote an article which revealed the very real truth i.e. Sexual or Sex Problems are common in both men and women, but more common in men. Thankfully, we are no longer living in the stone age and hence we can discuss and talk about sex more openly, about our sex lives, and what we do and do not want in the bedroom. After all, everyone deserves a great sex life – even you.

Common Sex problems prevailing in Men.

The most common problem amongst men is impotency or erectile dysfunction. It is the inability to maintain an erection good enough to complete sexual activity satisfactorily be it masturbation, oral or vaginal. There are two main causes – physical and psychological. Main physical causes includes diabetes, atherosclerosis, alcoholism, side effect of certain drugs, spinal cord injury, prostate gland surgery, etc while psychological causes includes relationship conflicts, stress and anxiety, depression, sexual boredom etc.

The only way to prevent it is to have healthy food habits, no smoking, no alcohol consumption; regular exercise and low fat diet. In case any of the above problems noticed, please consult a doctor and if you are on treatment for a medical or psychiatric problem you must consult specialist to solve it. The treatment can include oral or injectable medicines, mechanical aids, hormonal replacements or psychological therapies.

THE SECOND COMMON SEX PROBLEM IS LACK OF LIBIDO, WHICH MEANS LESS INTEREST IN SEX OR LOSS OF SEXUAL DESIRE. THE CAUSES FOR IT CAN BE NATURAL, PHYSICAL OR PSYCHOLOGICAL LIKE RELATIONSHIP DIFFICULTIES, SEXUAL BOREDOM WITH A PARTNER, DEPRESSION AND STRESS OR LOW LEVEL OF TESTOSTERONE.

Other problem in males is premature ejaculation, which occurs quickly during penetration itself or very soon afterwards. The treatment can be sex therapies or do-redo methods e.g. men should stimulate the penis near the point of ejaculation and then stop for 30-60 seconds and again stimulate it. Repeat the process for five to six times. Stimulation to penis should be done near the point of ejaculation and then firmly squeeze around the penis just below the head. It has effect of preventing ejaculation. Or else local anaesthetic gel or spray will reduce the penile sensation. Sometimes, antidepressant drugs are also prescribed when a man has some religious reason for not touching the penis with his hand. But before you go for any drug, please always consult the specialist doctor. Some of the tips are being mentioned here under to reduce the premature ejaculation :

CONDOM, &/OR ANAESTHETIC SPRAY WILL REDUCE THE SENSITIVITY OF PENIS
DON’T ATTEMPT INTERCOURSE UNTIL YOUR PARTNER HAD AN ORGASM
INCREASE FREQUENCY OF EJACULATION [MASTURBATION] THIS MIGHT HELP TO DELAY SUBSEQUENT EJACULATIONS.
COMMUNICATE WITH YOUR PARTNER
DO STRESS RELIEVING EXERCISE
FIRM DETERMINATION WILL ALSO HELP YOU TO CONTROL EJACULATION.

Delayed ejaculation is another form of problem, which is a root-cause of bad effects in your relationships. It is the inability to ejaculate. There are basically due to both physical and psychological reasons:

Physical causes:

AGE – AGING IS ASSOCIATED WITH DECREASED PENILE SENSITIVITY
DRUGS – BECAUSE OF SIDE EFFECTS ANTIDEPRESSANT OR ANY OTHER DRUG
HORMONE IMBALANCE – IT CAN ALSO LEAD TO DELAYED EJACULATION.
NERVE DAMAGE

Psychological causes:

PERFORMANCE ANXIETY
SELF – CONSCIOUSNESS
STRESS
RELATIONSHIP DIFFICULTIES

Treatment can be done by the following ways:

RELAXATION EXERCISE
USE OF VIBRATOR
SEX THERAPY

Avoid penetration till the point of ejaculation.

CHECK WITH DOCTOR IF YOU ARE ON ANY MEDICATION

Please always follow healthy guidelines for better sex life and to prevent any kind of sex-related problems. Some tips are as under:

Healthy food habit and supplements:

Obesity is a major problem in sexual dysfunction, diabetes and hormones. To keep yourself fit avoid refined carbohydrates, sugar, oil, fatty food etc and take more of protein [chicken, fish for non vegetarians], vegetables, salads, milk-products etc.

DIETARY SUPPLEMENTS LIKE MULTIVITAMIN OR MULTI MINERALS.
VITAMIN E IMPROVES BLOOD VESSELS OF PENIS AND ALSO OF HEART. 400-800 IU VITAMIN E PER DAY WILL HELP IN NORMAL ERECTILE FUNCTION
ARGININE – IT INCREASES PRODUCTION OF NITRIC OXIDE [WHICH EXPANDS BLOOD VESSELS] – AND AS A RESULT HELPS IN MAINTAINING ERECTION. DOSAGE IS 1000 MG TWICE A DAY.
TESTOSTERONE – THIS HORMONE MAY INCREASE SEXUAL LIBIDO AND ACTIVITY.

Quit smoking / alcohol:

Tobacco is having hazardous molecules called free radicals which is associated with in/sub fertility

AVOID WEARING TIGHT UNDERWEAR
REGULAR EXERCISE.
HEALTHY LIFE STYLE

Pain during Intercourse also causes disturbance in happy and healthy sex life. In normal circumstances, these can be because of

PEYRONIE’S DISEASE- DUE TO THIS YOU MIGHT ALSO OBSERVED CURVE OR BEND IN THE PENIS WHEN IT IS ERECT.
IRRITATION OF THE SKIN ALONG THE SHAFT OF THE PENIS
INFECTION OF THE PROSTATE GLAND OR URETHRA

In either of the cases, you should contact the specialist

Pain during ejaculation or pain in pelvis can be cause of infection of the urethra or sexually transmitted disease.

Dry Orgasm

A dry orgasm is a sexual climax that does not result in ejaculation that is no discharge of semen. The semen back flows into bladder rather than emerging through penis. This condition is called “Retrograde Ejaculation”. The possible causes for this condition are Diabetes, Prostate surgery, Certain medicines or Damage to the Prostate or seminal Vesicles. This condition leads to infertility. In this scenario, the doctors recover the sperm from unrine and use in artificial insemination.

Size of the Penis

Average length of penis is 4 to 8 inches [10-20 centimetres. Penis size can be increased and there are two procedures, medical and surgical procedures. Medical Procedure:

BY REPEATED STIMULATION
FREQUENT MASTURBATION
FREQUENT SEX
MASSAGE WITH CREAM, OIL ETC
VACUUM PUMP

Surgical Procedure:

FAT INJECTION
PENILE AUGMENTATION SURGERY

Common Sex Problems in Females

Lack of libido in women

Approximately more than 1/3rd females are affected by this problem. There might be physical or psychological factor affecting the sexual life for women and other number of reasons which causes disturbance in sexual life. Some of them are:

DRUGS/MEDICINES:BIRTH CONTROL PILLS, MOOD STABILISERS, TRANQUILLISERS AND ANTI-DEPRESSANT DRUGS HAS AFFECT ON SEXUAL DESIRE.
DAILY RESPONSIBILITIES:AS WOMEN PLAYS MULTIPLE ROLE IN HER LIFE LIKE, MOTHER, DAUGHTER ETC., SHE IS OFTEN TIRED WITH DAY TO DAY ACTIVITIES AND AS A RESULT WOMEN LOST INTEREST IN SEX.
EMOTIONAL ISSUES :TO RULE OUT THE POSSIBILITY, WOMAN MUST CONSULT A SPECIALIST DOCTOR HAVING DIFFICULTY IN ORGASM IS ANOTHER FREQUENT PROBLEM FACED BY WOMEN. APPROXIMATELY 24% WOMEN ARE AFFECTED BY THIS AND AS A RESULT STOP TAKING INTEREST IN SEXUAL ACTIVITIES WITH THEIR PARTNERS. PAIN DURING INTERCOURSE IS COMMON; AND 15% WOMEN ARE AFFECTED. IF WE GO INTO THE DETAILS, THERE ARE MAINLY TWO FACTORS:

PHYSICAL & PSYCHOLOGICAL FACTORS

PHYSICAL FACTORS ARE HORMONAL IMBALANCE AS WOMEN EXPERIENCE A RANGE OF SEXUAL RESPONSES FOR MARRIAGE TO CHILD BIRTH TO MENOPAUSE. IN EVERY STEP THERE ARE CHANCES OF HORMONE IMBALANCE. THE OTHER CAUSES INCLUDE INFECTIONS, DIABETES, MULTIPLE SCLEROSIS AND SIDE EFFECTS AND CERTAIN DRUGS.

PSYCHOLOGICAL FACTORS INCLUDES CULTURE, FAMILY, PERSONAL EXPERIENCES, JOB WORRIES, ANXIETY, PRESSURE OF JUGGLING DAY TO DAY WORK AND FAMILY, DEPRESSION, PAST SEXUAL ABUSE OR RAPE. MANY FACTORS PLAYS – ROLE FOR SEXUAL PROBLEM. EVEN ONE TIME PSYCHOLOGICAL TROUBLE CAN CREATE BIOLOGICAL OR PHYSICAL PROBLEM AND VICE VERSA. IT IS BETTER TO CONSULT A DOCTOR AT THE EARLIEST.

As far as Diagnoses are concerned, doctor will begin with physical exam, symptoms, pelvic examination and Pap smear. Counsellors will look into other possibility for psychological factors such as fear, anxiety, relationship problems, past sexual trauma or abuse etc. Depending upon the cause, treatment starts accordingly.

For treatments, Doctors, Counsellors and Sex therapist first see the root-cause in the problem. If the cause is physical or biological, doctors treat accordingly. Like for e.g. If a lady has a pain during sex, it can be reduced by applying lubricants. Psychological cause can be treated by providing education, enhancing stimulation, proving sexual behaviour therapy and encouraging non-sexual behaviours. The success of treatment depends upon the cause of the problem. Problems related to stress, fear or anxiety can be successfully treated with counselings, education and improved communication between partners. Biological problems, needs attention of specialist doctors and with proper treatment it can be cured.

Role of hormones in women’s sexual function

Hormones play a critical role in regulating sexual function. Decrease level of Oestrogen related to ageing and menopause and with that women might experience dryness in vagina and less sensation. Low level of testosterone is often related to problems like decline in sexual arousal, genital sensation and orgasm.

Mind processes sexual information to the body – which is poorly understood and why we respond to sexual feeling, moods, pictures, videos and books are also a complex phenomena. Following are the hormones required for good sexual life.

TESTOSTERONE:IT IS NECESSARY FOR SEXUAL FEELINGS AND RESPONSE.
OESTROGEN IS MADE FROM TESTOSTERONE AND WOMAN HAVING NORMAL MENSTRUAL CYCLE CONFIRMS THE OESTROGEN AS WELL AS TESTOSTERONE. IT IS NECESSARY FOR THE GENITAL RESPONSE AND FOR BREAST RESPONSE TO SEXUAL STIMULATION. IT IS REQUIRED FOR BLOOD SUPPLY TO VAGINAL WALL AND AS A RESULT VAGINA SECRETES A LUBRICATING FLUID.
PROTECTION:PITUITARY GLAND PRODUCES PROLACTIN AND HIGH AMOUNT OF PROLACTIN IS ASSOCIATED WITH ABSENT OR IRREGULAR PERIODS OR DIFFICULTY IN CONCEIVING.
THYROID HORMONES: CORRECT AMOUNT OF THIS HORMONE IS REQUIRED FOR SEXUAL FEELINGS AND RESPONSES. THE COMMON SEXUAL PROBLEM WILL BE LESS LIBIDO AND LOWERED SEXUAL RESPONSE IN WOMEN WITH UNDER ACTIVE THYROID GLANDS,
